These reports may include information regarding driving record, criminal history, social-security-number trace, and other public records.

You have the right under the Fair Credit Reporting Act (FCRA) to request a free copy of your report from the CRA, to dispute information contained in the report, and to receive a summary of your rights. If adverse action is considered, you will be notified in advance with a copy of the report and a summary of your FCRA rights.

By signing below, I authorize The Parking Company Worldwide and Checkr, Inc. to obtain and share consumer reports about me for employment purposes. This authorization remains valid throughout my employment unless revoked in writing.
